Thank you for entrusting us with your care. We look forward to serving you. Please arrive 15-20 mins prior to your initial appointment to complete paperwork, or complete your paperwork online using the detailed instructions below.

BEFORE YOUR APPOINTMENT – Complete your profile and new patient paperwork online by following these easy steps:

1. Go to the Patient Portal website using this link
2. Login using the email address you provided to the clinic and your temporary password (check your email for this password).
3. Once you login, you should change your password for increased security. Please note, your temporary password will expire 48 hours after you receive your email welcome letter. You may use the Forgot Password button to request a new password if your temporary password has expired.
4. Add your demographic information by choosing Profile from the navigation menu.
5. Next, fill out your intake paperwork by choosing Records, followed by Patient Forms from the navigation menu.
6. Fill out each page and select “I’ve read and accept this form” after each form has been completed.
7. Finally, add your signature to complete the intake paperwork process.
